Changes of Address
If you wish to advise us of a change of address, and you have registered for our on-line services, you are able to send us the details via this website. Just click on the 'Change Address' tab highlighted in orange and fill in the details. This will need to be done for each patient registered.
New Patient Registration
New patients can complete their 'Pre-Registration' form on-line and send it to us via this website. You will still need to come into the surgery with your NHS number to enable us to complete your registration. When registering, please don't forget to ask for your 'on-line' registration details to enable you to access on-line booking of appointments and ordering of your repeat medication.
