You will not be able to access on-line facilities from Friday 08 June 2012, but this will be restored shortly after 'going live'.  Appointments can only be booked on-line up to this date.

You are still able to book appointments by phoning or coming in to the surgery.

We ask that you will be patient while we are 'learning' what to do.

Waddesdon Surgery serves the whole of Waddesdon as well as its surrounding villages.

We offer a full general practice service and specialist services. In addition we have an onsite dispensing service.

We aim to treat all our patients promptly, courteously and in complete confidence.

This website is for existing patients and those considering registering with us and gives general information about the practice.

If you live in our practice area and would like to register, there are two ways of registering:

1. You may complete the 'Pre-registration form' on-line (one for each person), but please omit the 'lifestyle questionnaire' as we do require you to complete the practices own form.  This can be downloaded here or collected from reception as we do require you to come into the surgery to sign your form to complete your registration.
2. Alternatively ask for a registration form at reception in the surgery.

You will be registering with the practice rather than an individual GP, but you may see whichever doctor you prefer to.

As a newly registered patient you are invited to attend a new patient check with one of our practice nurses.

***NEW*** Patients registered for the on-line service are able to advise us of a change of address via the website. Click on the button on the left and complete the form. A form is required for each patient.
